I don't stick to any one structure or layout for my blogs. Some have just a few pages while others have hundreds. Remaining flexible and viewing each blog as it's own entity is the way to go. Concerning your theme question... personally I get bored using the same theme so I like to mix it up. But the theme is something I don't concentrate much on at all in the first few months, I generally just choose one of the latest ones in the wordprses admin Add New Themes page (under the Newest or Featured links, this pulls them straight from the official Wordpress themes database).
